If there was an option of using someone else I think I would, not sure I would trust them again. On the other hand, the owner has put measures in place to ensure it can't happen again. It must have been awful for you though, and poor Kody, imagine if he had been very little.....
The Dog Walker was a no show :(
I came home to find that my Kodybear was crated allllllll day, from 8:30am-5:30pm and no one came to let him out to eliminate and exercise. I was absolutely furious. I immediately took him outside to pee and stretch. Were going for a nice long walk once my partner comes home. I called the walker to find that there was a mix up in the schedule. She was very apologetic, the owner held herself accountable for the mix up and implemented a plan of checking in with the girls every morning to ensure no puppy/dog is missed. She even offered to give me a free visit for the month.
Obviously Kody is fine and he even held it all day with no accidents. What would you do? Should I give a second chance or should I jump ship and find another dog walker?

I agree with everyone else and would give them one more chance as they now know you are going to be watching them so are more likely to give you a great service as don't want a bad reputation. In the meantime I would look into another dog walking service as a back up just in case they let you down again
